• BLS - Basic Life Support
• ACLS - Advanced Cardiac Life Support
  • Job Details
  • Must have MA license in hand for acceptance
  • 1-year plus experience in Case Management, LTAC/Rehab
  • CM experience must be specific for hospital discharge planning (rehabilitation hospital experience a plus but not required)
  • Familiar with EMR and/or documentation expectations for role (ACMA SOP)
  • Familiar with Continuity of Care / Transition of Care requirements (CMS standards)

    • Experience in hospital discharge planning
      • Schedule Information
      • Shift: Mon-Friday 8-5p
      • Shift is 5x8 days but any variation to 8 hour shift (staying late/coming in early) has to be discussed and approved by manager
